NEW SOUTH WALES
Sydney Local Health District Aged Care & Rehabilitation Services
Aged Chronic Care & Rehabilitation Services focus on older people with multiple or complex medical problems and care needs. We provide assessment services to enable clients to access a variety of aged care services along with delivering clinical and therapy services in inpatient, community and home settings.
Australian Nursing Home Foundation (ANHF)
ANHF is one of the largest Chinese Aged Care organisations in Australia, aiming to provide culturally appropriate care for the sick, elderly and needy people of the Australian Chinese community.
Carers NSW
Carers NSW is the peak non-government organisation for carers in NSW and a member of the National Network of Carers Associations. Our focus is on improving the lives of carers. Carers NSW works with all carers regardless of their age, location, life-stage or circumstances. This includes those caring for family members or friends with support needs relating to ageing, disability, health and mental illness.
Co.As.It - The Italian Association of Assistance
Co.As.It’s Aged Care Team provides one-on-one support and assistance to the older Italian community by way of advocacy and case work as well as working at a policy and community development level to address the needs of the community with Government Departments and mainstream organisations.
Council on the Ageing NSW (COTA)
COTA NSW empowers people through advocacy and information to create an age-inclusive NSW for people over the age of fifty.
Dementia Australia
Dementia Australia a unified, national peak body for people, of all ages, living with all forms of dementia, their families and carers and the new voice of Alzheimer’s Australia. Providing support services, education and information.
Federation of Ethnic Communities Councils of Australia Inc. (FECCA) 
FECCA is the peak, national body representing Australians from culturally and linguistically diverse backgrounds. FECCA's role is to advocate and promote issues on behalf of its constituency to government, business and the broader community.
 
Partners in Culturally Appropriate Care (PICAC) NSW & ACT.
PICAC seeks to equip residential and community aged care service providers to deliver culturally appropriate care to older people from culturally and linguistically diverse communities.
Greek Welfare Centre (GWC)
The GWC provides services and programs that empower individuals and groups from the Greek community, and the community at large to participate in all aspects of Australia’s Culturally and Linguistically Diverse society.
Healthdirect Australia
Healthdirect Australia is a national, government-owned, not-for-profit organisation. They have been helping Australians manage their health and wellbeing for over a decade through a range of telehealth and digital health information and advice services.
Inner West Area Sector Support & Development (SSD)
SSD provides a range of support, resource and information services to CHSP/CCSP funded service providers, including: information and advice about government policy; planning; representing the local network on relevant committees; producing brochures and other materials for and about services.
Macarthur Disability Services (MDS)
Macarthur Disability Services (MDS) provides a range of services for people with a disability, their families and carers for those in the Macarthur Region and South West Sydney area.
Primary Health Networks (PHNs)
PHNs is an Australian Government health initiative with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
Seniors Rights Services
Seniors Rights Service is an advocacy service for people living in residential aged care or receiving in-home care. They offer free and confidential support  and promote the rights of older people to aged care service providers and community members. Seniors Rights also offer a free and confidential legal services for all related issues plus planning ahead and financial elder abuse.
The NSW Elder Abuse Helpline
The NSW Elder Abuse Helpline offers a free service that provides information, support and referrals relating to the abuse of older people living in the community across NSW. They place the safety of the older person first. The service is confidential and callers remain anonymous. Anyone can make the call.

ABRISA
ABRISA is the Brazilian Association in Victoria that welcome and support the newcomers to make them feel at home; orient to seek information from specific trusted sources; celebrate our cultural heritage; teaches about our culture and language; take care of our seniors.
COTA
COTA Victoria is a not-for-profit organisation that has represented the voice of older Victorians (aged 50 plus) since its establishment in 1951. The organisation represents all older people, however ‘people living in poverty, in inadequate housing and in social isolation, and older people experiencing abuse have always been on COTA's action agenda and will continue to be a focus into the future.

Jewish Care supports and enhances the wellbeing of the Jewish community of Victoria by providing excellent support and care for individuals and families.
Macedonian Community Welfare Association
MCWA is a statewide not-for-profit community based organisation. We respond to the growing welfare based needs of Macedonians by providing services to Macedonians by Macedonians as well as support others in their delivery of services to Macedonians.

Mecwacare is a not-for-profit organisation that provided care to the Victorian community for over 58 years, offering residential aged care, home care packages, respite care, in-home support, community housing, community programs, disability and nursing services.
MiCare
MiCare is a culturally and linguistically diverse organisation which began as DutchCare, providing Dutch heritage-centric care for migrants from Holland, now growing old in Australia. But we have spread beyond that beginning, partnering and growing to embrace the New Hope Foundation, Help At Home and other initiatives.  
PRONIA
PRONIA is a community services agency that serves the Greek speaking community and contributes to the wider multicultural sector through advocacy and collaborative partnerships. PRONIA’s programs focus on direct service delivery, including counselling, information provision, crisis intervention, community education as well on the delivery of specialist services in aged care, disabilities, family and children’s programs.
 Spectrum
Spectrum delivers services to newly arrived and established people with migrant and refugee backgrounds in the northern and western Melbourne areas. 
We help new Australians feel at home and we work together with individuals and families to help them successfully settle into all aspects of community life. Spectrum provides a full lifecycle of services including Home Packages, In Home Care and Respite, NDIS Services and a diverse range of Settlement Programs and Case Management.
 
Victorian Arabic Social Services (VASS)
VASS is an incorporated not-for-profit association providing support to people of Arabic-Speaking-Background (ASB) communities in Victoria, including counselling, case work, referrals, advocacy, community development, mediation, and social, cultural and linguistic support to people of ASB and other culturally and linguistically diverse backgrounds.
